Home Delivery Pharmacy Eligibility Requirements

Qsymia is indicated for patients 12 years of age and older. However, you must be 18 years or older to register for the Qsymia Engage Home Delivery Program and receive the requested product. If you are between 12-17 years of age, a caregiver must sign up on your behalf.

You must have a valid email address.

The Qsymia Engage Home Delivery Pharmacy Program is a cash-only program and does not process insurance claims. Patients or caregivers MAY NOT SUBMIT any purchase made through the Home Delivery Pharmacy program for reimbursement to any state or federal government healthcare program, and it cannot be applied towards my true out-of-pocket expenses.

You must have a prescription for Qsymia submitted to Medvantx by your Healthcare Provider.

You must have a valid physical mailing address in the United States, Puerto Rico, Guam or the U.S. Virgin Islands.

The Qsymia Engage Home Delivery Pharmacy Program requires a valid method of payment, credit or debit card, at time of product shipment. Checks or cash are not acceptable means of payment.

Free Standard Shipping on 90-Day Web Orders

As of April 1, 2020 standard shipping and handling is now FREE on all 90-day web orders placed via the Qsymia Engage Home Delivery Pharmacy. 90-day prescriptions are not permitted in all states. Check with your healthcare provider.
